Sliding Fee Scale  (Income Based - Limited Availability)
My practice offers a sliding fee scale on a limited basis to those in financial hardship, without insurance, or to those denied access to out-of-network benefits.

Insurance Reimbursement Policy
Most insurance companies will reimburse for services offered at Berscheid Counseling & Consulting.  Tom Berscheid is fully licensed by the state of Minnesota as a Marriage & Family Therapist, and Licensed Professional Counselor.  Berscheid Counseling does not bill your insurance provider directly, however.  A receipt for services provided is given at the session and will include: 1) your payment amount;  2) diagnostic codes;  3) insurance codes for session;  4) number of sessions provided.  The Client is responsible to obtain, fill out and submit reimbursement forms directly to their insurance provider.  Berscheid Counseling cannot guarantee insurance reimbursement, as this is solely determined by one’s own insurance policy and provider contract.  The Client agrees that payment (regardless of insurance reimbursement) is the sole responsibility of the Client.
